The research team identified candidate compounds by comparing gene expression changes induced by Alzheimer's disease with the effects of 1,300 FDA-approved drugs. After selecting five promising drugs, they confirmed that the letrozole-irinotecan combination was most effective.
Discovery Driven by Big Data Analysis
Researchers analyzed single-cell gene expression data from Alzheimer's patient brains and cross-referenced it with the Connectivity Map database, which contains cellular responses to thousands of drugs. Additional analysis of electronic health records from 1.4 million patients aged 65 and older revealed that patient groups taking certain medications showed lower rates of Alzheimer's disease.
Professor Marina Sirota stated, "Alzheimer's causes complex changes in the brain that have made research and treatment difficult, but computational tools allowed us to directly address this complexity. We are encouraged by the discovery of potential combination therapy based on existing FDA-approved drugs."
Distinct Actions on Neurons and Glial Cells
Experimental results showed that letrozole primarily acts on neurons, while irinotecan acts on glial cells. Mice receiving the combination therapy showed reversal of gene expression changes that occurred during disease progression, reduced accumulation of toxic proteins, and decreased brain damage. They also demonstrated significant improvement in memory tests.
Professor Yadong Huang noted, "It is very encouraging that computational results were confirmed in a widely used Alzheimer's mouse model. Clinical trials in Alzheimer's patients will proceed soon."
Future Prospects [AI Analysis]
This research may hold significance for Alzheimer's treatment in three key aspects.
First, the drug repurposing strategy—reusing existing approved drugs instead of developing new ones—could substantially shorten the time to clinical application. Since letrozole and irinotecan are already safety-validated drugs, early clinical phases can be bypassed to focus on efficacy verification.
Second, approaches that simultaneously modulate multiple pathways rather than single targets may gain prominence. Since Alzheimer's arises from complex brain changes rather than a single cause, combination therapies targeting both neurons and glial cells simultaneously may prove more effective.
Third, big data and AI-based drug discovery methodologies are likely to become the new standard in neurodegenerative disease research. The approach used in this study—integrating patient records, genetic data, and drug databases—can be applied to other conditions such as Parkinson's disease and ALS.
However, whether success in mouse models translates to human patients must be verified through clinical trials. Challenges remain, including neurological side effects of cancer drugs, long-term safety with extended use, and determining appropriate dosages.
